Guidance for primary care and specialist colleagues referring patients to Dr. Eaddy for interventional pain care — including patients with implanted stimulators and pain pumps.
Conditions treated & procedures offered
Dr. Eaddy is a double board-certified interventional pain management physician (ABIPP; ABIM). The practice evaluates and treats acute, chronic, and post-operative pain — including back and neck pain, sciatica, neuropathy, arthritis, headache, complex regional pain syndrome (CRPS), and failed back surgery syndrome — and offers injections and nerve blocks, spinal cord stimulation, dorsal root ganglion (DRG) and peripheral nerve stimulation, targeted drug delivery (pain pumps), and device management and revision.

What records and imaging to send
To make the first visit productive, please send relevant office notes, a current medication list, prior pain interventions and their results, and any pertinent imaging (X-ray, MRI, CT) — ideally the report and access to the images. For patients with an implanted device, include the device make, model, and implanting provider if known.

Referral fax & insurance authorization
Fax referrals and records to (904) 683-5688. For questions about referrals or to coordinate authorization, call the main office at (904) 453-7976. Please include the patient's insurance information so staff can begin any prior authorization.
What turnaround should referring clinicians expect?
Staff review a referral after the requested records and insurance information arrive, then contact the patient to arrange the appropriate visit. Timing varies with clinical urgency, record completeness, insurance authorization, and appointment availability. For a time-sensitive referral or implanted-device concern, call (904) 453-7976 rather than relying on fax alone.
Referring patients with an existing stimulator or pain pump
The practice accepts patients who already have a spinal cord stimulator, DRG or peripheral nerve stimulator, or intrathecal pain pump — including devices placed elsewhere — for reprogramming, troubleshooting, refills, and revision. Note the device type and manufacturer on the referral so the visit can be scheduled appropriately.

When to refer urgently
Please call the office directly for prompt scheduling if a patient has a device alarm or alert, suspected device-site infection (redness, warmth, drainage, fever), a sudden loss of pain control, a suspected medication-pump issue, or new neurological symptoms. For any medical emergency, the patient should call 911.
