How it's used

Contact form submissions are used solely to respond to your inquiry, by the contact method you select. We do not sell, rent, or share your information with third parties for marketing.

What not to send

The contact form is not a secure channel for medical information. Please do not submit detailed medical history, medication lists, insurance numbers, or other sensitive health information. For medical matters, call the office at (904) 453-7976.

This site does not use advertising cookies or third-party tracking.

Email newsletter

If you subscribe to our patient education newsletter, we collect only your email address. You must confirm the address by clicking a link we send you before any newsletter is delivered. We use it solely to send occasional patient education updates. We do not sell, rent, or share this list, we do not connect it to any medical record, and subscribing does not create a physician–patient relationship. Every email includes an unsubscribe link that takes effect immediately.
