Information we collect

This website collects only the information you choose to submit through the contact form: your name, email address, phone number, and your message. All four are required to send the form. Standard technical logs (such as IP address) may be processed by our hosting provider for security and performance.

How it's used

Contact form submissions are used solely to respond to your inquiry. Office staff respond by phone during business hours. We do not sell, rent, or share your information with third parties for marketing.

To deliver your message, the contact form uses a small number of service providers. Cloudflare hosts this website, runs the form, and uses Cloudflare Turnstile, which receives your IP address and browser information, to help block automated spam. Resend sends your submission to the practice by email, and the practice's email is hosted by Apple iCloud. The website itself does not keep a separate copy of your submission.

What not to send

The contact form is not a secure channel for medical information. Please do not submit detailed medical history, medication lists, insurance numbers, or other sensitive health information. For medical matters, call the office at (904) 453-7976.

Cookies and analytics

This site does not use advertising cookies. We use Cloudflare Web Analytics to count page visits in aggregate (for example, which pages are viewed, which site a visitor came from and the type of browser), and a simple counter on this site that records how often visitors tap a phone number or a contact button and on which page. These tools do not use cookies and do not record your name, contact details or message, or anything you type into the contact form. Pages on this site also load fonts from Google Fonts, and the home page and the testimonials page show Google reviews through Featurable; these requests share your IP address and browser information with those providers.

Email newsletter

If you subscribe to our patient education newsletter, we collect only your email address. You must confirm the address by clicking a link we send you before any newsletter is delivered. We use it solely to send occasional patient education updates. We do not sell, rent, or share this list, we do not connect it to any medical record, and subscribing does not create a physician–patient relationship. Every email includes an unsubscribe link that takes effect immediately.
