Preparing for Your First Visit
What to bring, how to send records ahead, and what to expect — so your first appointment moves straight to what matters: finding the source of your pain.
In short — Call (904) 453-7976 to schedule, have your records and imaging faxed ahead to (904) 683-5688, and bring your ID, insurance cards, a complete medication list, and — if you have one — your implanted device's card and remote. The first visit is about diagnosis: history, examination, imaging review, and a plan you understand.
How to prepare, step by step
- Schedule by phone. Call the main office at (904) 453-7976. Staff will confirm which of the three locations to visit and let you know anything specific to bring.
- Send records ahead. Ask your referring clinician to fax office notes, imaging reports, and prior treatment records to (904) 683-5688 — see the referring clinicians page for exactly what helps most.
- Gather your imaging. Bring reports — and discs, if you have them — for any X-ray, MRI, or CT related to your pain.
- List your medications. Every medication and supplement, with doses. Note any blood thinners — they matter when planning procedures such as injections.
- Bring ID and insurance cards. Staff will verify coverage and begin any prior authorization your plan requires.
- Bring device information. If you have a spinal cord stimulator, DRG or peripheral nerve stimulator, or a pain pump, bring the device card, the remote or charger, and the name of the implanting provider.
- Write down your pain story. Where it hurts, when it started, what makes it better or worse, what you've tried, and the questions you want answered.
What to expect during the visit
An accurate diagnosis comes first. Dr. Eaddy will take your history, perform a focused examination, and review your imaging with you before discussing treatment. Plans start with the least invasive effective option and advance only as needed — from medication management and image-guided injections to radiofrequency ablation and neuromodulation. You should leave the first visit understanding what is likely causing your pain and what the next step is.
Do I need a referral?
Many patients are referred by their primary care physician or a specialist, but you can also call the office directly — staff will let you know what your insurance requires and help coordinate records.
Will I have a procedure at the first visit?
The first visit focuses on diagnosis and planning. Any procedure is scheduled separately, with instructions specific to that procedure — including whether to pause blood thinners and whether you will need a driver.
What if I already have a stimulator or pain pump?
Patients with existing devices — including devices implanted elsewhere — are welcome for reprogramming, refills, troubleshooting, and revision. See device troubleshooting & revision for what the practice manages, and bring your device card and prior records.
